Job Description – Prescription Assistance Coordinator
Hours per week –40
Reports to – Executive Director, Shelby Community Health Center
Job Overview
To oversee a consistent Patient Assistance Program (PAP) for the Health Center, to support enrollment of
SCHC patients who are eligible to participate in the PAP on a timely basis and to provide case
management and administrative support.
Duties and Responsibilities:
•Continue to develop and oversee a consistent Center-wide process for the PAP program.
•Provide a variety of case management and administrative support.
•Assist patients with completing PAP applications and renewals as well as tracking follow-up
requirements.
•Verify patient financials as needed.
•Submit applications for pharmaceutical companies and document information appropriately and
timely.
•Coordinate medication regimes with prescriber and patient.
•Maintain and ensure up-to-date and accurate medication profiles.
•Act as a resource to patients to address and resolve inquiries and problems regarding
medications.
•Act as a resource to process triage calls regarding medications as needed.
•Assist in the maintenance of the medication room. Track and log all samples in and out of the
room ensuring accurate inventory.
•Assist with access to sample medications.
•Perform charting as appropriate.
•Maintain statistics and prepare reports regarding program participants and pharmaceuticals.
•Collect, organize, and keep up-to-date all PAP procedures, resource materials and
forms/applications for each manufacturer of medications that has a PAP.
•Research best practices regarding free clinics and patient assistance programs
•Develop and manage volunteers to assist with PAP.
•Maintain patient confidentiality as required by law.
•Other duties as assigned.
Required skills:
•High School Diploma or equivalent, plus one year relevant experience.
•Must also have one year of general office experience.
•Experience in health care, pharmacy or non-profit organization a plus.
•Excellent customer service, communication, and interpersonal skills.
•Excellent organizational skills and problem solving skills.
•Detail-oriented and ability to prioritize workload.
•Able to work effectively with patients, SCHC staff and board members
•Ability to investigate and resolve problems.
•Works well independently as well with others in a team setting.
•Proficient in use of MS Excel and MS Word, database software a plus.
